Working Out Nonstop but Still Struggling to Build Muscle? Let’s Talk Peptides.

You show up. You lift. You sweat. And the mirror barely changes. Here's what we look at first, and where peptides fit into the conversation.

I hear this in clinic every week, from Westlake moms training five days a week to guys at the Golds gym in Bee Cave who haven't missed a workout in years: "I'm doing everything right. Why isn't my body changing?"

Usually something is getting in the way, and it's often not the workout. Lately a lot of patients come in asking about peptides. That's a fair question. But before we get there, let's look at the bigger picture.

Working hard is not the same as building muscle

Muscle grows when you train it, feed it, and let it recover. Miss any one of those and progress slows down. Training is only the signal. The growth happens afterward.

The usual reasons muscle stalls

  • Not enough protein. Most patients I see eat less than they think, especially at breakfast.
  • Under-eating overall. A big calorie deficit makes it hard to add muscle. Patients on GLP-1 medications need extra attention here.
  • Too much cardio, not enough load. Classes are great for your heart. Muscle needs progressive resistance.
  • Poor sleep. Short or broken sleep hurts recovery and appetite control.
  • No rest days. More is not always better. Muscle repairs between sessions.
  • Hormone and thyroid changes. Shifts in testosterone, estrogen, and thyroid function with age can make muscle harder to build and keep.

Age plays a part

We naturally start losing muscle in our 30s and 40s, and the pace can pick up in perimenopause, menopause, and with lower testosterone. That doesn't mean you can't build muscle. It means the basics matter more, and it may be worth checking your labs.

So, let's talk peptides

Peptides are short chains of amino acids, the building blocks of protein. Your body makes many of them naturally, and they act as signals between cells. You've probably seen them all over social media, often with big promises attached.

Here's my honest take:

  • Peptides are not a shortcut and they don't replace training or protein.
  • They are not right for everyone, and some people should not use them.
  • Products bought online without a prescription may not contain what the label says.
  • The only place to decide is with a licensed provider who knows your health history and labs.

If you're curious, we're happy to talk through it at a consultation. We'll look at your goals, your health history, and your bloodwork, and tell you honestly whether peptides make sense for you or whether something else deserves attention first. You can read more on our peptide therapy education page.

What we check first

  • Labs. Hormones, thyroid, vitamin D, iron, and metabolic markers can all affect energy and recovery.
  • A DEXA body composition scan. The scale can't tell muscle from fat. DEXA estimates lean mass and body fat, so your next scan shows whether you're actually gaining muscle.
  • Hormone support when it's appropriate. For some patients, hormone replacement therapy is part of the plan.
  • Your routine. Protein, sleep, training split, and recovery. Small changes here often do the most.

The bottom line

If you're working out nonstop and not seeing results, your effort isn't the problem. You may just be missing a piece of the picture. Let's find it together.

Frequently asked questions

Why am I not building muscle even though I work out a lot?

The most common reasons are not enough protein, not enough recovery, poor sleep, too much cardio relative to strength work, under-eating, and hormone or thyroid changes. A provider visit with labs helps sort out which ones apply to you.

Can peptides build muscle on their own?

No. Nothing replaces training, protein, sleep, and recovery. Peptides are a topic we may discuss at a consultation after we review your history and labs, and they are not right for everyone.

Do you prescribe peptides online without a visit?

No. Any peptide discussion happens with a licensed provider during a consultation, after a health history and appropriate labs.

How can I tell if I am actually gaining muscle?

The scale cannot tell muscle from fat. A DEXA body composition scan estimates lean mass and body fat, so a repeat scan shows whether your training is building muscle.
