Telehealth Informed Consent

Telehealth involves the delivery of healthcare services using electronic communications between a patient and a clinician who are not in the same physical location. This consent explains how Body Tonic RX provides telehealth, the benefits, risks, and your rights.
Nature of telehealth services
- Asynchronous review of intake forms, photographs, and lab results.
- Synchronous (live) audio or video consultations when clinically required or requested.
- Secure electronic messaging with your care team.
- Electronic prescribing and coordination with licensed compounding and dispensing pharmacies.
Expected benefits
- Convenient access to licensed clinicians without travel.
- Continuity of care for ongoing wellness, hormone, and weight-management needs.
- Faster follow-up and adjustment of treatment plans.
Risks and limitations
- Information transmitted may be insufficient (e.g., poor image resolution) to allow appropriate clinical decision-making.
- Delays in evaluation or treatment may occur due to technology failures.
- Security protocols can fail, causing a breach of privacy of personal health information.
- In rare cases, the clinician may determine the condition is not appropriate for telehealth and refer you to in-person care.
Patient responsibilities
- Provide accurate, complete, and current health information.
- Notify us promptly of changes to medications, conditions, or adverse reactions.
- Follow your clinician's instructions and complete recommended labs and follow-ups.
Prescribing
A prescription is issued only when, in the clinician's professional judgment, it is appropriate and medically necessary. We do not prescribe controlled substances via telehealth. There is no guarantee that a consultation will result in a prescription.
Emergencies
Telehealth is not appropriate for medical emergencies. If you are experiencing chest pain, shortness of breath, suicidal thoughts, severe allergic reactions, or any other emergency, call 911 or go to the nearest emergency room.
Right to withdraw consent
You may withdraw consent at any time without affecting your right to future care, by notifying us at info@mybodytonic.com.
