HIPAA Notice of Privacy Practices

Effective: January 1, 2026

This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Uses and disclosures of PHI

  • Treatment — to coordinate care among clinicians, pharmacies, and labs.
  • Payment — to process payment for services rendered.
  • Healthcare operations — quality improvement, audits, training, and credentialing.
  • As required by law — public-health reporting, abuse reporting, judicial orders, and similar.

Your rights

  • Inspect and obtain a copy of your medical record.
  • Request amendment of inaccurate records.
  • Receive an accounting of disclosures.
  • Request restrictions on certain uses and disclosures.
  • Request confidential communications by alternative means or at alternative locations.
  • Receive a paper copy of this Notice on request.
  • Be notified of any breach of unsecured PHI.

Our responsibilities

We are required by law to maintain the privacy and security of your PHI, abide by the terms of this Notice, and notify you in the event of a breach.

Complaints

You may file a complaint with us at privacy@mybodytonic.com or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
