The Functional Medicine Labs Your PCP Won't Order (And Why You Still Feel Awful)

You told your doctor you're exhausted by 2 p.m., gaining weight around your middle, losing hair in the shower, and sleeping like a toddler on espresso. They ran a CBC, a basic metabolic panel, maybe a TSH — and said the words that make women in Austin want to scream into a pillow: "Everything looks normal."
Here's the truth: your labs weren't abnormal. They were incomplete.
Why your PCP won't order these labs
It's usually not neglect. It's the system:
- Insurance medical necessity rules. Insurers reimburse tests that diagnose disease, not tests that optimize function. "Fatigue" rarely unlocks a full thyroid panel or fasting insulin.
- Fifteen-minute visits. Root-cause work takes 45–60 minutes of history and pattern recognition. Primary care isn't built for it.
- Reference ranges are population-based, not optimal. "Normal" is a bell curve of mostly unwell Americans. Being in the bottom 3% of normal is still normal — and still miserable.
- Screening vs. optimizing. Your PCP's job is ruling out disease. Functional medicine asks a different question: why is this system drifting, and how early can we catch it?
The labs we actually order — and what they reveal
1. A complete thyroid panel (not just TSH)
TSH is a pituitary signal, not a measure of thyroid function at the tissue level. We look at Free T4, Free T3, Reverse T3, and TPO/thyroglobulin antibodies. Antibodies can be elevated for years before TSH budges — that's early Hashimoto's, and it explains cold hands, constipation, thinning brows, dry skin, and stubborn weight.
2. Fasting insulin, C-peptide, and HbA1c together
Glucose is the last thing to break. Insulin rises first — often 5–10 years earlier. Fasting insulin above single digits with a "normal" A1c is the classic pattern behind belly fat, afternoon crashes, sugar cravings, and PCOS-type symptoms.
3. A full sex hormone panel with timing that matters
Estradiol, progesterone, total and free testosterone, SHBG, DHEA-S, LH/FSH — drawn at the right point in your cycle. Perimenopause commonly starts in the late 30s with normal FSH. Rage, 3 a.m. wake-ups, anxiety that feels chemical, and a shorter fuse are hormonal data points, not personality flaws.
4. Iron studies and ferritin (not just hemoglobin)
You can be years into iron depletion with normal hemoglobin. Ferritin under 50 is a well-known driver of hair shedding, air hunger with exercise, restless legs, and brain fog — especially in menstruating women.
5. Vitamin D, B12, folate, magnesium RBC, and homocysteine
Nutrient status governs energy production, methylation, mood, and immune resilience. Serum B12 can look fine while functional markers say otherwise.
6. Inflammation and cardiometabolic risk: hs-CRP, ApoB, Lp(a), homocysteine
Standard cholesterol panels miss particle burden. ApoB and Lp(a) are far better predictors of cardiovascular risk — and Lp(a) is largely genetic, so testing it once changes your prevention plan for life.
7. Cortisol rhythm
Wired at night, wrecked in the morning is a rhythm problem, not a willpower problem.
8. Body composition, not just body weight
A DEXA scan shows lean mass, fat mass, and visceral fat. Two women at the same weight can have completely different metabolic futures — and if you're on a GLP-1, protecting lean mass is the whole game.
Symptoms that deserve a deeper look
- Fatigue that sleep doesn't fix, or a hard 2–4 p.m. crash
- Weight gain around the midsection despite unchanged habits
- Hair shedding, brittle nails, dry or crepey skin

- Night sweats, 3 a.m. wake-ups, new anxiety or irritability
- Low libido, painful sex, cycle changes
