Right now, more than 7 million Americans are living with Alzheimer’s. Nearly two-thirds of them are women. If you’re a woman over 40, your lifetime risk of developing Alzheimer’s is estimated to be about 1 in 5.

When estrogen declines in perimenopause and menopause, it doesn’t just affect hot flashes and mood.
Estrogen:

The “30% Lower Risk” Data
Some large observational studies have shown that women who begin hormone therapy within about five years of menopause may have up to a 30% lower risk of developing Alzheimer’s disease compared to women who never used hormones.
Timing appears to matter.
Starting HRT earlier during the window when the brain is still responsive may offer protective benefits. Starting much later (like after 65) does not appear to offer the same protection and may carry different risks.
This is called the “critical window” or “timing hypothesis.”
Is HRT a magic bullet? No.
Is it something we should stop dismissing? Absolutely.
Because here’s the truth: Alzheimer’s begins decades before symptoms show up. By the time someone is diagnosed, the process has often been unfolding for 15–20 years.
So if you’re 45, 50, 55 you are not early. You are right on time.

Hormone therapy is not for everyone. It is not a “take this and you’ll never get dementia” promise.
It is a highly individualized decision based on:
- Cardiovascular health
- Metabolic health
- Family history
- Inflammation
- Personal risk factors
- Type, dose, and delivery method of hormones
This is why I get frustrated with 7-minute doctor visits and blanket “no” answers.
Women deserve nuance.
Women deserve data.
